Keyworking alongside professionals
The West Yorkshire Keyworker Service works alongside existing networks of care/treatment in a non-clinical way. We provide children/young people and families/carers with practical support to navigate services. When a child/young person is in crisis there can be multiple professionals involved, this can be daunting for a family with each professional having a specific remit or area of work.
How we work alongside you as professionals
The keyworker works alongside professionals who support children/young people and their family/carers by offering specific support and help navigating services. The keyworker ensures the children/young people and their families are at the centre of discussions and planning regarding the care they are receiving. The keyworker uses a non-clinical approach – starting at where things are for the child/young person and listening to their views and wishes. We work closely with professionals to establish where the keyworker can compliment the existing package of support.
How to introduce children, young people or young adults to the service
If you assess that the individual may need additional support, a review in their care or meet the criteria for a keyworker, you should consider a referral to the Dynamic Support Register (DSR). (Please note DSR’s have criteria which would also need to be met).
The care co-ordinator (or any other relevant professional) can refer the child/young person on to their local authority Dynamic Support Register. Click here to find contact info for local area DSR leads.
The individual will need to meet the criteria for the local area’s DSR.
The Chair and representatives of local services on the panel will review the risk using a risk rating tool.
If the criteria for the DSR and the Keyworker Service is met, please contact us to get a copy of the referral/introduction form.
The referral will be triaged and a telephone/teams call will be booked with the referrer. This is to share key information about the child/young person and their parent/carers needs and identify the role for a keyworker.
The child/young person will be discussed in our weekly management allocations meeting to be allocated a keyworker.
You will then be contacted to inform you of the allocated keyworker.
Please note that consent must be obtained for this referral to take place.

In West Yorkshire, there are DSRs for each of the 5 areas. In each area there is a DSR for under 18’s and a separate one for adults 18+. The is a mix of learning disability and autism or autism only registers and meetings. They take place weekly, fortnightly or monthly depending on area. At this meeting, referrals to the West Yorkshire Keyworker Service will be discussed. Children, young people or their parent/carer can self refer to the DSR.
